Tax-Child Abuse or Neglect / Criminal Record and I-9 Forms

All paid and volunteer staff members must have 3 references, an application and
a child abuse or neglect/ criminal record form.

All paid staff members must also fill out and send in tax forms and a I-9 form with
copies of either their birth certificate/ passport or copies of their drivers license
and social security card.

You can fill the printable application out on your computer or go to our

online application (link is below) and fill

out

NOTE: You CANNOT save the online application. If you experience a power surge or
power failure you will loose all your information and will need to start over from the
beginning. You CAN save the printable application document as a word document.

You can save the printable application as a word document at anytime.
When you have finished the application and have saved it, attach it as a file to

an e-mail directed to me at
