 Register For Summer Camp

We are so glad that you are interested in enrolling your child in Sherwood Forest Camp.  Below is the paperwork we will need you to fill out, print, sign, and mail to our office.  If you do not have access to a printer, just call and we will put information in the mail to you.  Spaces in each program are limited, so please don’t delay in returning your child’s application for the summer of 2010.

 

  

Step 1:

Read and review the Frequently Asked Questions or the Parent/Camper Handbook.

Step 2:

Complete the application.  Don’t forget to select the session you would like your child to attend.

Please note all Leadership Training Programs are full for the summer of 2010.  Please feel free to complete an application and have your child placed on a wait list.

Step 3:

Print, sign, and mail the application with your $25 deposit to Sherwood Forest Camp, 2708 Sutton Blvd., St. Louis, MO 63143

Your child will be placed on a waiting list without a completed application & deposit!

We will contact you within two weeks of teh receipt of your application to confirm receipt of the application, ask any additional questions, and/or set-up an interview between the camper and a staff member.

Step 4:

Fill out the complete health history and Summer Food Service Program (SFSP) Income Eligibilty Form after February 15, 2010.

Step 5: 

Locate and copy your recent tax return (or other income verification) and your child’s health insurance card (front and back).

Step 6:

Print, sign, and mail completed forms, income verification and health insurance card to Sherwood Forest Camp, 2708 Sutton Blvd, St. Louis, MO 63143.

We will confirm that your child’s application is complete via phone call, mail, or email and confirm the session and bus stop location.

Step 7:

Mail any remaining fees by April 20, 2010 to Sherwood Forest Camp, 2708 Sutton Blvd, St. Louis, MO 63143.  If your child is enrolled after this time, all forms and fees are due at the time of enrollment.  Please call if this presents a difficulty for your family.

Your child’s enrollment is not complete until ALL FORMS AND FEES are returned to the office

Step 8:

Take another moment to review the Frequently Asked Questions or the Parent/Camper Handbook.

Step 9:

   

Bring your child with his/her belongings (don’t forget to use a permanent marker to label them with the child's name!) to the designated bus stop at the designated time on the first day of their session.
